Comprehensive Case Management

How it works

Families are referred by the Board of Social Services, previous guests, clergy, other service agencies and self-referral. When a potential guest makes the initial phone call, a preliminary assessment is performed by the Case Management Team to determine eligibility. If the family is eligible for emergency shelter services and there are openings, an in-person interview is scheduled.

During the interview, the Case Management Team reviews extensively guest policies, procedures and expectations. Together, the Case Manager and the family develop a Success Plan that establishes the goals and action steps the family will accomplish while they are in the program. Case Management sessions are required weekly to monitor progress on goals and help families overcome barriers to self-sufficiency 

Family Promise provides supportive services tailored to meet the individual needs of each respective family unit. Our Case Management Team coordinates the service delivery for families to obtain identification, birth/marriage certificates, health insurance, immunizations, child support, childcare, school enrollment, transportation, parenting classes, voter registration and other vital resources necessary to promote the long-term goal of self-sufficiency and permanent housing. 

But our goal is not simply getting families into housing; it is keeping them in housing.
